Vaccinations for Your Clinic
Clinics, pharmacies, and hospitals interested in administering COVID-19 vaccine must enroll in the federal COVID-19 Vaccination Program. The Washington State Department of Health is coordinating this enrollment process for providers in Washington state. You can use the COVID-19 Provider Enrollment Guide (PDF) to help you through the enrollment process.
Check Your Eligibility
If you have a private practice or are unable to receive the vaccination through your workplace, you can use the Washington State Department of Health’s PhaseFinder tool to confirm your vaccine eligibility.
The PhaseFinder tool allows you to input personal details like age, location, and you work in health care. At the last screen, you will be prompted to enter your name to get a confirmation message to show to a vaccine provider to demonstrate your eligibility.
